Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll discuss your dental concerns, collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.

Your time is precious, and we know you probably have a lot on your plate already before adding in a trip to the dentist. To help your check-in process go a little faster at your first visit, you can complete our new patient forms from home. Just download them from the button below, fill them out, and hand them to our front desk staff when you get here!

If you’re unable to complete them ahead of time don’t worry. We just ask that you please arrive 10-15 minutes early so you have time to fill them out before your scheduled appointment.

Whether your first appointment is for a dental checkup, a consultation, second opinion, or emergency care, we do all we can to make your visit a welcoming, informative, and productive experience. Once we gather the relevant background information, examine your smile and obtain any diagnostic images, we’ll discuss if treatment is required and explain your best options in care.
